According to U.S. Centers of Medicare & Medicaid Services, home health care encompasses the full extent of health care services that can be given in your home. The need for these services can be any ailment or cognitive and functional impairments. Home health care is generally inexpensive, suitable, and equally successful comparatively to the care received in an acute care setting (hospital based) or long term care (skilled nursing facility).

Skilled home health services sample cases include:

• Surgical wound or pressure sores (Skilled Wound Care Management)
• Patient and caregiver teaching and training with return demonstrations
• Intravenous Solutions and Medications Therapy; Nutrition Therapies: (Enteral Nutrition: Gastrointestinal (GI) tract to transport part or all patient’s caloric need; Parenteral Nutrition: Transporting calories and nutrients through a vein
• Clinically observe and assess critically ill and volatility of health level or status
• Injections

Our skilled home health services are provided by qualified professionals comprising of but not restricted to Registered Nurses, Certified Home Health aides, Speech/Language Pathologists, Doctors of Physical Therapists/Physical Therapists, and Occupational Therapists. The duration of skilled home health intervention is based on the evaluating clinicians’ plan of care goals for the patient. The frequency of skilled home health visits is based on the requirements triggered by the patient’s illness or injury. The patients/clients must show a level of improvement to continue skilled service interventions.

Our patients/clients are recertified after every 60-day episode requiring skilled care services. Medicare does not restrict the number of recertification episodes for patients/clients who continue showing progress and have ongoing skilled service eligibility for home health benefits.

Patients/clients are required to receive certification from their doctor/PCP that states they are in homebound status due to illness or injury. You will be under the care of a physician who is a Medical Doctor (i.e., a Doctor of Osteopath, a Doctor of Pediatric Medicine, etc.). A PCP must complete a FACE-TO-FACE ENCOUNTER (F2F) and CERTIFY that you have a SPECIFIC SKILLED NEED for home health services.

Homebound status may be determined in the following circumstances:

• The patient needs the aid of assistive devices such as crutches, canes, wheelchairs, and walkers
• The patient needs the use of special transportation or the assistance of another person in order to leave their place of residence
• The patient has a medical condition such that leaving his or her home is medically contraindicated
• The patient has a normal inability to leave home as it requires a considerable taxing effort
• The patient has a residual generalized weakness
• The patient has a gait abnormality or difficulty in walking

24/7 care is not covered by Medicare Home Health Care Benefits.

Compassionate Home Health will not follow patients/clients when they are admitted to the hospital. Once you are discharged from the hospital and receive an order from your doctor/PCP for Resumption of Care, then we will set up an appointment to meet you at your home to complete an assessment of your home health care needs.

Unfortunately, no. Skilled home health services are strictly provided only to patients/clients at their personal homes.

No. Medicare Home Health Benefits does not cover prescription medications.

Durable medical equipment (i.e hospital bed, cane, walker, and crutches) must prescribed by the doctor/PCP. Upon approval, Medicare covers 80 percent (%) under the umbrella of Medicare Part B qualifications guidelines.

No, Medicare does not pay for home health personal care services.

Once patients/clients meet the home health criteria, they will continue to be Medicare eligible for home health care while under hospice care for the medical conditions/diagnosis not associated with the terminal conditions/diagnosis.

• Compassionate Home Health accepts Medicare and Medicaid, but, unfortunately, we cannot accept Commercial/Private Insurances (i.e., Kaiser, Aetna, Blue Cross) at this time.
• If your Medicare coverage is through a Medicare Health Plan, please check with your plan to know your Medicare-covered Home Health Benefits.
• If you have a Medicare Supplement Insurance (Medigap) policy or any other health insurance coverage, please let your doctor/PCP and Compassionate Home Health know so your medical bills will get paid accurately.

• Your Primary Care Physician (PCP) orders are crucial and required for the Start of Care (SOC). When your PCP prescribes home health services for you, Compassionate Home Health will call you to set up an initial consultation and meeting in your home. We will have some SOC questionnaires (i.e., ID Verification; Admission Service Agreement and Consent Form; Medicare Secondary Payer Form Screening; Emergency Phone Numbers and Instruction to Call form; and Home Safety Evaluation) to complete in order to admit you to Compassionate Home Health services. We will discuss your needs thoroughly and, together, come up with a set of goals. Then the skilled clinician (RN, PT, ST) will set up the frequency and duration (length of services and other add-ons of services you might need) based on that initial assessment.
• Compassionate Home Health personnel will contact your PCP or physician who referred you to begin home health services to discuss your plan of care. We will continuously update your doctor on your current progress in the skilled program (i.e Occupational Therapy, Registered Nursing, Physical Therapy, Home Health Aide, and Registered Dietitian, Speech Therapy).
• It’s imperative that Compassionate Home Health personnel/staff see you for home visits per the frequency and duration ordered by your doctor or PCP.

• Assess and clinically observe what you’re eating and drinking.
• Assess and clinically observe your blood pressure, temperature, heart rate, and breathing.
• Closely monitor and clinically observe that you’re compliant in consuming your day-to-day prescription medications and other medicines and performing any additional treatments accurately.
• Inquire if you’re experiencing or complaining of pain.
• Assess, clinically observe, and closely monitor your safety in and out of the home.
• Teach and train you about your plan of care so you can be functionally independent and take care of yourself at home safely with low to no risks of falling.
• Systematize your care. Compassionate Home Health will report frequently to you, your doctor/PCP, and any other staff/personnel who attend to your skilled care/needs.
