- Which multidisciplinary professional will provide skilled services for you?
- What’s the frequency and duration you will need skilled home health services?
HOW TO QUALIFY FOR HOME HEALTH SERVICES?
- A patient must be HOMEBOUND, and a doctor must clarify that you are homebound.
- Under the care of a physician who is a Medical Doctor (i.e., Doctor of Osteopath, Doctor of Pediatric Medicine)
- A doctor must complete a FACE TO FACE ENCOUNTER (F2F) and CERTIFY that you need a SPECIFIC SKILLED NEED for Home Health Services.
